Guardianship
A guardian is someone appointed by the court to make decisions for one who has been determined unable to make decisions on their own. These decisions typically revolve around living arrangements, healthcare, arranging benefits, and other items deemed necessary. As a court-appointed guardian, I work with you or your loved one to create a comprehensive, person centered care plan to meet immediate and long term needs. I include my clients in all decisions, big and small, to the fullest extent possible. I go to doctor's appointments and advocate to ensure my client is being well care for and receiving proper supports. I conduct frequent visits at my client's residence to ensure their arrangements are meeting their needs and make any adjustments necessary.
Care Management
Sometimes people need assistance to make the proper arrangements as they age or due to a disability. Perhaps family members live out of state or are unable to commit the time to helping. Care management includes an assessment that identifies areas where help is needed and we will work with you and your loved ones to put a comprehensive, person-centered plan in place to meet your care needs. This may include assistance setting up benefits, finding service agencies to help clean, cook meals, and more. I can provide assistance in finding out of home residential care, if necessary. Care management is broad and varies greatly depending on the situation.
Medical Power of Attorney
Sometimes a person knows they need help and are cognitively capable of making the the decision to appoint a Medical Power of Attorney. As Medical Power of Attorney (MPOA), I work closely with you to ensure I am aware of your wishes for medical intervention should you become unable to make these decisions on your own. I work to capture your wishes into your advanced directive documents and distribute the directives to your physicians and others deemed necessary. As MPOA, I can also provide care management as needed. This can involve helping you or your loved one with many aspects of care, including arranging a new living situation, scheduling, transportation, and accompanying to medical appointments, and other coordination to ensure your life runs smoothly. Every situation is unique, services are tailored to meet your specific needs.
